ICD-10 coding for Wound Care (Non-Pressure)

Dear Mrs. Smith,

As you may have heard, our current ICD-9 codes will be replaced on October 1, 2013. Although this may seem far away, this will have a huge impact on podiatric physicians. There are hundreds and hundreds of codes, but we will focus specifically on codes pertaining to podiatric wound care. This is the first of several blogs, so all subsequent blogs will start with the title “ICD-10 coding…” followed by the specific topic to be addressed.

The new ICD-10 codes for ulcers are essentially divided into “Non-pressure Chronic ulcers” and “Pressure ulcers.” Podiatrists will find codes for “Non-pressure chronic ulcers” under the L97 category. L97 is defined as a “Non-pressure chronic ulcer of lower limb, not elsewhere classified” and includes:

  1. Chronic ulcer of skin NOS
  2. Non-healing ulcer of skin
  3. Non-infected sinus of skin
  4. Trophic ulcer NOS
  5. Tropical ulcer NOS
  6. Ulcer of skin NOS

AND excludes:

  1. Pressure ulcer (pressure area) (L89.-)
  2. Skin infections (L00. – L08.)
  3. Specific infections classified to A00.-B99.

 

The L97 categories are divided by anatomical location:

L97.2 - Non-pressure chronic ulcer of calf

L97.3 - Non-pressure chronic ulcer of ankle

L97.4 - Non-pressure chronic ulcer of heel and midfoot, including plantar surface of midfoot

L97.5 - Non-pressure chronic ulcer of other part of foot, including ulcer of toe

L97.8 - Non-pressure chronic ulcer of other part of lower leg

L97.9 - Non-pressure chronic ulcer of unspecified part of lower leg

 

The L97 codes are further broken down to the level of tissue damage, for example:

L97.42 – Non-pressure chronic ulcer of left heel and midfoot

L97.421 – Non-pressure chronic ulcer of left heel and midfoot - limited to breakdown of skin

L97.422 – “      “          “          “          “          “          “          “ – with fat layer exposed

L97.423 – “      “          “          “          “          “          “          “ – with necrosis of muscle

L97.424 – “      “          “          “          “          “          “          “ – with necrosis of bone

L97.429 – “      “          “          “          “          “          “          “ – with unspecified severity
